RISK MANAGEMENT

How to complete the RISK MANAGEMENT requirement for Coaches:

1) Go to Eastern New York Youth Soccer website - Risk Management link by clicking here.

2) Click on Long Island (LIJSL)

3) Login and register

4) Once you are approved, print out 3 copies of the approval notice

5) Give 1 copy to the Director of Safety, Frank Wolf, safety@eastislipsoccer.com

6) Keep a copy for your own records

7) Keep an extra copy to use when you register your child for the new soccer season

8) There is a $15.00 fee for this. Once paid, Coaches may deduct that fee from the cost of their child’s registration for the next season by turning in that extra copy with their registration.

9) Approval is for 2 years and then must be renewed in order for you to continue coaching.

RISK MANAGEMENT DISCOUNT
Procedure during on-line registraton process:
At the check-out screen make one of these two selections:
  1. Pay in full - After you have completed your transaction you can just mail the Risk Management receipt  with your check payment minus the $15 fee. (Not per child) . If you want to pay in full by credit card, we will then issue you a $15 refund when we receive your Risk Management receipt.

  2. Make a payment - This will allow you to just make a deposit on your registration. You can then send me the risk management form by mail or e-mail and I will adjust your account by the time you make your final payment either by check or credit card.
